Original superhero identity and their first appearance: Bucky debuted in Captain America Comics #1 (March 1941)
One or more reasons why he assumed different superhero identities: James has matured quite a bit since his days as Captain America's kid sidekick. So much so, in fact, that for a significant period of time, he adopted his mentor's identity.
Subsequent identities and their first appearance: Winter Soldier debuted in Captain America #1 (January 2005) / Captain America debuted in Captain America #34 (March 2008)

One or more reasons why he assumed different superhero identities: The gamma radiation that initially changed Bruce Banner into the Hulk, combined with a fractured personality brought about by the abuse inflicted on Bruce by his father, has resulted in numerous transformations throughout the years, some good, some bad, and some different.

Although, unlike Clint (allegedly), his motivations were occasionally aroused by a mental disorder.
Subsequent identities and their first appearance: Giant-Man debuted in Tales to Astonish #49 (November 1963) / Goliath debuted in The Avengers #28 (May 1966) / Yellowjacket debuted in The Avengers #59 (December 1968) / Wasp debuted in Secret Invasion: Requiem #1 (February 2009) / Ultron debuted in Avengers: Rage of Ultron #1 (June 2015)

Now I could go on and list some more Marvel characters who've had more than one superhero identity, such as Steve Rogers (Captain America/Nomad) and Carol Danvers (Captain Marvel/Ms. Marvel), but no, I think I'll leave that for a later date (along with my selection of DC characters who've done a similar thing).
